Shaping a Healthier Future
From 1 April 2021 expectant changes to the law will make it easier for the different parts of the NHS, such as hospitals, GP practices and community services, to work more closely with each other and local councils, charities and other organisations that provide health and care services.
Locally, health and care organisations are already working closely together as an Integrated Care System called Bath and North East Somerset, Swindon and Wiltshire (BSW) Partnership.
During 2021, BSW has worked closely with all of the leaders in health and care services, service users and those who deliver care from the NHS, local authority and third sector to determine how services that are more sustainable, accessible and more coordinated can meet the needs of local people. A health and care model has been produced which describes what health and care can look like for people for the next 10 years, and provide a framework to ensure that a coordinated approach is taken when planning and designing services across partner organisations.
The aim is for health and care to become much more than just providing services when people are unwell or injured. The aim is to focus on helping everyone to live a healthy lifestyle and to stay well.
Between 2 November 2021 and 14 December 2021 the BSW Partnership is asking people to complete an online survey in order to share their views on show to make health and care better in the local area.
